As a case management at Centura Health, you'll advocate for patients while collaborating closely with interdisciplinary health care partners, facilities and families to ensure the best possible care experience from admission through discharge and beyond by connecting patients to resources that help them maintain and build on positive outcomes.

Utilizes clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination, utilization management, and patient advocacy. Responsible for assuring medical appropriateness criteria are met for status and level of care.

What You Bring/Job Requirements

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

  • 1 year of nursing or case management experience, required.

  • Experience working with EMR, preferred.

  • Working knowledge of regulatory requiremetns and accreditation standards, preferred.

  • Associates Degree in Nursing required; Graduate of an accredited school of Nursing required

  • BSN Preferred

  • RN License required

  • BLS preferred

Physical Requirements - Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)

Schedule:FT Days-1.00/80 hours per pay period

Shift:8a-4:30pm and 8:30a-5p staggers shifts- works 2-3 weekend shifts in a 6 week period. Rotates holidays. Will do discharge planning on oncology/trauma unit, very high pace-
