Performs patient observation, data collection related to patient care, administers medication, treatment and proceduresaccording to the physicians’ orders and resident care plan. Communicates patient findings to the supervisor. Assumesresponsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify andmeet the physical, emotional and spiritual needs of the patients. Oversees care provided by CNAs, Care Partners, QMAPsand/or other support staff. Promotes the optimal health, well-being and safety of the patient through use of the nursingprocess and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personalaccountability for relationship-based care, organizational mission, and core values.

What You Bring/Job Requirements

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

One year of previous LPN experience, preferredRequires knowledge in nursing and person-centered care conceptsAbility to demonstrate competency in the provision of individual care of patients based on the patients’ assessed age anddevelopmental needs.

HS Diploma

BLS through American Heart Association or American Red Cross

Progressive Care Center (PCC) is an assisted living facility that offers skilled nursing care (both short and long term), rehabilitation therapies and Alzheimer’s Care in a home-like atmosphere. PCC is located adjacent to St. Thomas More Hospital, a 25-bed acute care facility serving Fremont County and surrounding southern Colorado communities.
